Parents and student-athletes that are unable to attend the Sports Meeting should view the sports meeting presentation, read the Antigo School District Activity Code and take the Athletics and Activities Code Exam (see the three links below). To receive credit for the Sports Meeting, the parent and student-athlete together must score 80% or better on the exam. Student Driver Permission Form - This form is meant for students that are tutoring or involved in School-to-Work programs during the school day, driving to a site for Phy. Ed. Class (i.e. golf course, etc.) or driving to an alternate athletic practice site (i.e. cross-country run). The Driver Information should also be completed.
Student Ride Home with Parent Permission Form - This form is to be completed by a parent when they wish to transport their child to/from a co-curricular activity for which a bus is being used.
Student Permission Form Riding with Volunteer Driver/Volunteer Driver Form - The student permission form is to be completed by a parent to allow their son/daughter to ride with another parent/staff member/person to/from an event. The volunteer driver form is to be completed by the person driving the student/s and attached to the student permission form.
